Supported Living Services Counselor


Position Summary:

Supportive Living Services (SLS) Counselors provide “Personal Support Services and Training and habilitation Services” per Title 17, Section 54349 (a) and (b).

Duties:

SLS Counselors will assist the client with a variety of support services based on assessed needs. These services may include; but are not limited to:

  • Laundry; meal preparation; grocery shopping; cooking; kitchen clean-up; household cleaning; use of public transportation in the community; personal health and hygiene; self advocacy training; independent recreation and participation in the community; use of medical and dental services, and other community resources; community resource awareness (such as police, fire, or emergency help); home and community safety; moving into a home or apartment; locating and choosing suitable roommates if requested by client; acquiring home furnishings; disputes with landlord; emergency services; money management; making purchases; paying bills; dealing with government agencies; advocacy for client rights; utilizing community resources.
  • An SLS Counselor should be familiar with his/her assigned client’s IPP (Individual Personal Plan) goals and ensure that services provided to the client are directly related to those goals. SLS Counselor is responsible to advise supervisor of IPP to which he/she has been invited, and to bring all appropriate materials to the meeting. SLS Counselor is responsible for updating and informing Supervisor and Service Coordinator of any changes with clients IPP goals and needs.
  • An SLS Counselor is responsible for preparing and completing all hours assigned with clients. SLS Counselor is also responsible for completing all paperwork on a weekly basis, inputting this information into the ESPC database.
  • SLS Counselor is responsible for attendance at all staff meetings and other designated trainings. SLS Counselor is responsible for maintaining current CPR and First Aid Certifications in accordance with Region Center policy.
